Customer Loyalty Program

By completing this form I agree to the following terms and conditions of K-O-K Products' Customer Loyalty Program

  1. Our facility will provide K-O-K with up-to-date information regarding billing address and contact information;
  2. Our facility will reasonably monitor on-site sodium hypochlorite storage vessels and ensure, to the extent practicable, that no foreign matter is introduced into these tanks and that any damage to these tanks is reported to K-O-K Products as soon as possible;
  3. Our facility acknowledges that Sanamax Sodium Hypochlorite 12.5% ("the Product") is an EPA Registered Pesticide and that, as such, its storage and use are subject to regulation and that it is a violation of federal law to use the Product in a manner inconsistent with its labeling. 
  4. Our facility further acknowledges that the placement of any unregistered sodium hypochlorite product, or any product bearing an EPA Registration Number other than 8154-20001, could subject both our facility and the product supplier to civil liability for "misbranding" of an EPA Registered Pesticide.
  5. Our facility will use its best efforts to ensure that during the swimming pool season, the on-site storage tanks are monitored and requests for refills are called in to K-O-K allowing a reasonable time for delivery. Repeated requests for same day delivery may subject our facility to forfeiture of any pending credits obtained under the Customer Loyalty Program.
  6. Should our facility determine that it would like to terminate its vendor relationship with K-O-K Products, it will contact K-O-K to allow K-O-K to retrieve any tank or tanks owned by K-O-K that are situated on the customers premises. Failure to so notify K-O-K will subject customer to a charge for the cost of the tank or tanks left in place.

Payment of Incentives

  1. K-O-K will credit to the facility $25 for each $1000 of prior year sales. Credit shall be applied against the first invoice sent in the month of July, beginning with July 2005.
  2. Amounts short of the next $1000 milestone shall carry forward to the subsequent year - thus, if a customer purchased $1500 of product during 2004, the customer would receive a $25 credit against the initial July 2005 invoice and as of January 2005 would carry a balance of $500 into 2005.
  3. After the second full year of maintaining a customer/vendor relationship with K-O-K, inclusive of 2004, Customer shall be entitled to a $50 credit towards pool supplies and equipment or, at customer's option, shall be entitled to a new Taylor test kit (Model K-2005 or equivalent).
  4. REFERRALS: For any apartment manager, management company, or developer who refers an account for either a new facility or an existing facility supplied by another vendor in 2004, to K-O-K for swimming pool chemical supply, such referring entity shall be entitled to a one-time referral credit of: (a) $50 for any apartment or condominium facility that purchases $1000 or more in 2005; or (b) $25 for any apartment or condominium facility that purchases less than $1000 during calendar year 2005. All amounts to be applied against August 2005 invoices. VALID FOR 2005 REFERRALS ONLY.
